Patient Forms

What to expect at your first visit

During your initial visit, you will meet with Dr. Koylass so that a thorough and detailed review of your medical and dental history is completed. You will be asked several questions and it is pertinent that the information you report is accurate. For this reason, these forms are available on our website where you can view the information that may be asked of you, prior to your appointment.

For patients taking any medications, prescribed, over the counter or any herbal supplements, we recommend that you walk with a complete list of these drugs. Additionally, if you are under the care of a physician, please be able to provide the name and contact information for this person. This information will help us to understand you and your mouth better so that the best treatment can be recommended.

The initial consultation will take roughly 45-60 minutes and will include a comprehensive clinical and radiographic (x-ray) examination to assess your current periodontal status. A full mouth series of radiographs will be taken at that first visit, unless a current set is supplied by your general dentist. Our office uses digital radiography, which reduces the time and exposure to radiation. The office is designed to provide our patients with a comfortable and caring environment. All our staff are trained and follow current OSHA standards and guidelines for infection control.

Once the examination is completed, patients will be educated regarded the diagnosis and all the treatment options and recommendations will be discussed. If your case is particularly complex and requires consultation with other specialists, you will be guided accordingly to obtain the best treatment outcomes. The office will also provide follow-up correspondence with your referring doctor to ensure comprehensive care.

We value your time. Our office works by appointments. We would appreciate that you arrive on time so that the same level of attention that is given to you is afforded to all our patients.
